ABSTRACT

SUMMARY: Breathing is considered a vital function dependent on factors such as adequate permeability of the nasal route, which is linked to physiological functions, intellectual processes, and craniofacial growth. The aim of this study was to determine the changes in the craniomaxillofacial growth and bone development of Sprague Dawley rats subjected to permanent experimental unilateral nasal obstruction. Twenty-four newborn rats were used, randomized, and divided into experimental and control groups. The right nostril was obstructed, and weight, length, and Lee's index measurements were recorded at 8 and 16 weeks. Craniomandibular x-rays were taken of each animal, obtaining linear neuro- and viscerocranial measurements. Then, a biochemical analysis was performed to measure the alkaline phosphatase concentration. The results were analyzed in the SPSS software, performing a descriptive analysis, using a t-test for independent samples, comparing basal, cephalometric, and biochemical characteristics between the control and experimental groups, considering a significance range of 5%. When comparing the experimental and control groups, the variables length, weight, and Lee's index presented no significant differences. In the x-ray analysis, at 8 weeks, the Co-L1 and Co-Mn measurements were reduced, whereas the Ba-So increased, with significant differences. At 16 weeks, the L1-O, Po-Ba, and E-Mu measurements decreased; however, Co-Gn registered a greater value with significant differences. The alkaline phosphatase levels fell significantly at week 16 in the experimental group. In conclusion, the reduction of permanent nasal respiratory flow is related to modifications in facial growth at 8 and 16 weeks and to the reduction of alkaline phosphatases at 16 weeks.


La respiración se considera una función vital, dependiente de factores como la permeabilidad adecuada de la vía nasal, vinculada con funciones fisiológicas, procesos intelectuales y crecimiento cráneofacial. El objetivo de este estudio fue determinar los cambios en el crecimiento y desarrollo óseo cráneo maxilo facial de ratas Sprague Dawley sometidas a obstrucción nasal unilateral experimental permanente. Se utilizaron 24 ratas macho neonatas, randomizadas y divididas en grupo control y experimental. Fue realizada obstrucción nasal de la narina derecha y realizadas mediciones de peso, longitud e índice de Lee a las 8 y 16 semanas. Se efectuaron radiografías cráneomandibulares a cada animal, obteniendo medidas lineales de neuro y viscerocráneo. Posteriormente se realizó análisis bioquímico, para medir la concentración de fosfatasa alcalina. Los resultados fueron analizados en el software SPSS, realizándose análisis descriptivo, empleando prueba T para muestras independientes comparando características basales, cefalométricas y bioquímicas entre los grupos control y experimental, considerando un umbral de significancia de 5 %. Al comparar los grupos control y experimental, las variables longitud, peso e índice de Lee no presentaron diferencias significativas. En el análisis radiográfico, a las 8 semanas, las medidas Co-L1 y Co-Mn presentaron reducción, mientras que Ba-So aumentó, con diferencias significativas. A las 16 semanas, las medidas L1-O, Po-Ba y E-Mu disminuyeron, sin embargo, Co-Gn registró un mayor valor, con diferencias significativas. Los niveles de fosfatasa alcalina disminuyeron significativamente en la semana 16 en el grupo experimental. En conclusión, la reducción de flujo respiratorio nasal permanente se relaciona con modificaciones del crecimiento facial a las 8 y 16 semanas y con la reducción de ALK en análisis a las 16 semanas.

ABSTRACT

The purpose of this research was the tomographic evaluation of the Mandibular Buccal Shelf (MBS) in orthodontic patients with different vertical growth pattern. An observational, descriptive, cross-sectional and retrospective study was conducted. Tomographic images of patients aged 14 to 40 years were observed and a database was formed with those that met the inclusion criteria. The sample size was 10 for each group according to vertical growth pattern (hypodivergent, normodivergent and hyperdivergent). Then four zones of frequent insertion of extralveolar mini-screws were selected in the MBS, taking as a reference the mesial and distal roots of the first and second mandibular molar. When comparing the characteristics of MBS between vertical growth patterns, between sexes and hemiarchs, no statistically significant differences were found. However, when the characteristics of MBS were compared according to the reference root, it was found that there were statistically significant differences. The vestibular area to the distal root of the second mandibular molar presented the highest values in terms of angulation, height and thickness. There are no significant differences in the bone characteristics of MBS according to vertical growth patterns, sexes or hemiarchs. Angulation, height and thickness progressively increase from the vestibular bone of the mesial root of the first mandibular molar to the distal root of the second molar.


El propósito de esta investigación fue la evaluación tomográfica de la placa ósea mandibular (POM) en pacientes de ortodoncia con diferente patrón de crecimiento vertical. Se realizó un estudio de tipo observacional, descriptivo, transversal y retrospectivo. Se observó imágenes tomográficas de pacientes de 14 a 40 años de edad y se formó una base de datos con las que cumplían los criterios de selección. El tamaño de muestra fue de 10 para cada grupo según patrón de crecimiento vertical (hipodivergentes, normodivergentes e hiperdivergentes). Luego se seleccionaron cuatro zonas de inserción frecuente de minitornillos extralveolares en la POM, tomando como referencia las raíces mesial y distal del primer y segundo molar mandibular. Al realizar la comparación de las características de la POM entre patrones de crecimiento vertical, entre sexos y hemiarcadas no se encontraron diferencias estadísticamente significativas. Sin embargo, cuando se comparó las características de la POM según la raíz de referencia se encontró que había diferencias estadísticamente significativas. La zona vestibular a la raíz distal de la segunda molar mandibular presento los mayores valores en cuanto angulación, altura y grosor. No existen diferencias significativas en las características óseas de la POM según patrones de crecimiento vertical, sexos o hemiarcadas. La angulación, la altura y el grosor aumenta progresivamente desde el hueso vestibular de la raíz mesial del primer molar mandibular hacia la raíz distal del segundo molar.

ABSTRACT

As exostoses mais conhecidas são o Torus palatino e mandibular, que se desenvolvem a partir do crescimento benigno da cortical óssea, localizadas respectivamente na linha média palatina e superfície lingual de caninos e pré-molares, podendo ser unilaterais ou bilaterais. Esse trabalho tem como objetivo o estudo de um grupo familiar que apresentaram o Torus como característica em comum, buscando confirmar a presença e prevalência das características clínicas multifatoriais listadas na literatura, possibilitando o diagnóstico da etiologia e assim traçando um plano de tratamento individual, se necessário. Como metodologia, foi realizada uma triagem com cada membro familiar na clínica Odontológica da Faculdade Sete Lagoas - FACSETE, descartando a participação no estudo os membros que não apresentaram o Torus. Os indivíduos que apresentaram indicação da remoção cirúrgica foram encaminhados para clínica de cirurgia da própria instituição. Concluímos, portanto, com este estudo que fatores genéticos e ambientais colaboram como fator etiológico mais predominantes para surgimento do Torus no grupo familiar estudado... (AU)


The best known exostosis are the palatine and mandibular Torus, which develop from the benign growth of the cortical bone, located respectively in the midpalatal line and lingual surface of canines and premolars, and maybe unilateral or bilateral. This work aims to study a family group that had Torus as a common feature, confirming the presence and prevalence of multifactorial clinical features listed in the literature, enabling the diagnosis of etiology and thus outlining an in dividual treatment plan, if necessary. As a methodology, a screening was performed by each family member at the Dental Clinic of Facul dade Sete Lagoas - FACSETE, discarding the participation in the study for members who did not present Torus. Individuals who indicated surgical removal were referred to the institution's own surgery clinic. Therefore, we conclude with this study that genetic and environmental factors collaborate as the most predominant etiological factor for the emergence of Torus in the studied family group... (AU)


Las exostosis más conocidas son el Torus palatino y el Torus mandibular, las cuales se desarrollan a partir del crecimiento benigno del hueso cortical y están ubicadas respectivamente en la línea medio palatina y en la cara lingual de los caninos y de los premolares que pueden ser unilaterales o bilaterales. Este proyecto tiene como objetivo el estudio de un grupo familiar que presentó en sus características comunes el Torus, buscando confirmar la presencia y el predominio de las características clínicas multifactoriales listadas en la literatura, lo que hizo posible el diagnóstico de la etiología y diseñar un plan de tratamiento individual, caso necesiten. Como la metodología fue hecho una selección con los miembros de la familia en la clínica odontologica de la Faculdade Sete Lagoas - FACSETE, quitando de los estudios las personas de la familia que no presentaron el Torus. Los individuos que presentaron indicación de extirpación quirúrgica fueron enviados a la clínica de cirugía de la escuela. Concluimos con este estudio que los factores genéticos y ambientales colaboraron como los factores etiológicos más predominantes para la aparición del Torus en la familia analizada... (AU)

Aim: This study aimed to evaluate the correlation between vertical dimension of occlusion (VDO) and various facial measurements in a sample of Sudanese adults. Material and Methods: A total of 113 dental students (33 males and 80 females) with a mean age of 21.7±1.26 years were enrolled in this study. Different facial measurements including (Eye-Mouth, Eye-Eye, Eye-Ear, and Ear Height) were compared with two different measurements of VDO: N-Gn (from the tip of the nose to the tip of the chin), and Sn-Me (from the base of the nose to the bottom of the chin). Pearson's correlation coefficient test was utilized for the correlation between the measured parameters. A p-value of less than 0.05 was considered significant for all analyses. Results: A significant positive correlation was shown between all measured facial distances and both measured VDO distances. Though, the strongest correlation was seen for the eye-mouth distance (r= 0.725, p<0.001), while the weakest was for ear height (r= 0.254, p= 0.007). A paired t-test revealed a significant longer N-Gn distance than Sn-Me distance. Also, it has been shown that there were no significant differences between right and left sides of the face. Conclusion: The distance measured from the outer canthus of the eye to the angle of the mouth can be used to predict Subnasale-Menton (Sn-Me) distance.


Objetivo: Este estudio tuvo como objetivo evaluar la correlación entre dimensión vertical oclusal (DVO) y varias medidas faciales en una muestra de adultos sudaneses. Material y Métodos: Un total de 113 estudiantes de odontología (33 hombres y 80 mujeres) con una edad media de 21,7 ± 1,26 años se inscribieron en este estudio. Se compararon diferentes medidas faciales que incluyen (ojo- boca, ojo-ojo, ojo-oído y altura de la oreja) con dos medidas diferentes de DVO: N-Gn (desde la punta de la nariz hasta la punta del mentón) y Sn -Yo (desde la base de la nariz hasta la parte inferior del mentón). Se utilizó la prueba del coeficiente de correlación de Pearson para la correlación entre los parámetros medidos. Un valor de p inferior a 0,05 se consideró significativo para todos los análisis. Resultados: Se mostró una correlación positiva significativa entre todas las distancias faciales medidas y ambas distancias DVO medidas. Sin embargo, la correlación más fuerte se observó para la distancia ojo-boca (r=0,725, p<0,001), mientras que la más débil fue para la altura de las orejas (r=0,254, p=0,007). Una prueba de t pareada reveló una distancia N-Gn significativamente más larga que la distancia subnasal-mentón. Además, se ha demostrado que no hubo diferencias significativas entre los lados derecho e izquierdo de la cara. Conclusión: La distancia medida desde el canto externo del ojo hasta el ángulo de la boca puede utilizarse para predecir la distancia subnasal-mentón.

Desde la antigüedad la belleza y la armonía facial han sido objetivos que tanto pintores como escultores, médicos y las personas tratan de lograr. Los rostros han sido estereotipados y es así como las caras proporcionadas tanto en alto como en ancho con perfiles que son rectos o levemente convexos son los que se asocian a bondad, generosidad, personas con luz interior, princesas y son las "buenas de las películas". Pero ¿qué sucede con aquellos rostros cóncavos, con tercios inferiores ya largos o cortos?… a ellos les ha tocado la peor parte, son los malos, los oscuros, los "villanos de las películas". Objetivos La finalidad de este trabajo es acercar a los colegas ortodoncistas una opción de tratamiento en clase III esqueletal, cuando el camuflaje ortodóncico no alcanza para resolver el crecimiento alterado entre el maxilar superior y el maxilar inferior (AU)


Beauty and facial harmony have been, since the beginning of humanity, a target for artists, painters, sculptores, and even people and doctors to accomplish. There is a stereotype of beauty: faces with proportional dimensions both in height and length, with straight profiles or somehow convex are associated with kindness, generosity and inner beauty. We can see this in movies where the heroes, the princesses, the "good guys" are portrayed with these types of faces. And what happens with people with concave faces, long or short inferior thirds? They are doomed to be the evil, the dark and "the villains" (AU)

ABSTRACT

Objetivo: Describir el plan de tratamiento completo para el manejo de pacientes con fisura labio alvéolo palatina (F.L.A.P.), basado en nuestra experiencia clínica de más de 40 años. Casos clínicos: En nuestro protocolo consideramos fundamental el tratamiento ortopédico, teniendo en cuenta los tiempos de crecimiento y desarrollo del maxilar superior de cada paciente, para luego aplicar las técnicas quirúrgicas en un maxilar armónico, con un éxito más predecible. Se describirá una serie de casos clínicos con seguimiento de pacientes. Conclusión: Con este protocolo que pregonamos desde hace muchos años, nos diferenciamos principalmente de otras propuestas por considerar los tiempos biológicos de cada paciente en cuanto a crecimiento y desarrollo, y no por basarnos en tiempos quirúrgicos preestablecidos. Aplicando este protocolo obtenemos resultados predecibles que entendemos que solo son posibles de evaluar, al realizar el seguimiento del paciente hasta completar su desarrollo (AU)


Objective: To describe the comprehensive treatment plan aimed at managing patients with cleft lip and palate (CLP) on the basis of our more than forty years of clinical experience. Case reports: e orthopedic treatment is deemed fundamental in the present protocol, which takes into account the maxilla growth and development periods of each individual patient, to later perform a surgical technique in a harmonized maxilla, with a more predictable success. Clinical cases with the patient follow-up shall be described. Conclusion: e main difference between the present protocol that has been held for many years and others proposed approaches mainly lies in considering the unique growth and developmental biological stages of each patient and not in drawing on pre-established surgical timing. When applying this protocol, predictable results are achieved and they are only meant possible to be assessed during the thorough patient follow-up (AU)

ABSTRACT Introduction: over the years, populations have been studied by means of lateral cephalic x-rays in a variety of ethnicities, ages, and study types, setting standards for different groups. In Latin America, studies show cephalometric differences from standards based on Caucasian populations. Method: 1,627 cases of patients without prior treatment were analyzed; the sample included 855 males and 772 females aged 6 to 12 years. Lateral cephalic radiographs and specific tracing were taken. Descriptive analysis was done using mean, standard deviation, minimum, and maximum. Comparisons were made between male and female subjects and by age. Results: a greater size was found in all measurements in male subjects, being statistically significant in some measurements and ages. The ages with the most differences were 8 and 9 years, and the least difference occurred at the age of 10. Conclusion: there was variation in maxillary and mandibular size with age and gender, with the largest size in males and indications of vertical predominance.


RESUMEN Introducción: a través de los años se han estudiado poblaciones por medio de radiografías cefálicas laterales, en variedad de etnias, edades y tipos de estudio, lo que ha permitido establecer estándares para diferentes grupos. En Latinoamérica, los estudios demuestran diferencias cefalométricas respecto a las normas basadas en poblaciones caucásicas. Método: se analizaron 1.627 casos de pacientes sin tratamiento previo, 855 hombres y 772 mujeres entre 6 y 12 años. Se tomó radiografía cefálica lateral y un trazado específico. Se realizó análisis descriptivo utilizando media, desviación estándar, mínimo y máximo. Se hizo comparación entre los sujetos masculinos y femeninos y por edades. Resultados: se encontró mayor tamaño en todas las medidas en hombres, siendo estadísticamente significativas en algunas medidas y edades. La edad que mayor diferencia presenta es a los 8 y 9 años, y la de menor diferencia es a los 10 años. Conclusión: se presentó variación en el tamaño maxilar y mandibular con la edad y el género, con mayor tamaño en los hombres y con características de manifestaciones de predominio vertical.

ABSTRACT

Objetivo: Describir la resolución de un caso un odontoma complejo de gran tamaño con utilización de osteosíntesis rígida. Caso clínico: Una paciente de 13 años de edad se presentó a la consulta por aumento de volumen en hemifacia izquierda, de 4 meses de evolución. Mediante biopsia incisional, se diagnosticó odontoma complejo. Se realizó tratamiento quirúrgico con utilización de osteosíntesis rígida. Se decidió retirar la placa de osteosíntesis a los 6 meses posoperatorios para evitar alterar el crecimiento y el desarrollo mandibular. Se indicó control posoperatorio durante 5 años. Conclusión: La utilización de osteosíntesis rígida fue adecuada para reforzar el defecto óseo mandibular producido durante el tratamiento quirúrgico de un odontoma complejo de gran tamaño (AU)


Aim: To describe the resolution of a case of a large complex odontoma with the use of rigid osteosynthesis. Clinical case: A 13-year-old patient presented with a swelling in left jaw of 4 months of evolution. Complex odontoma was diagnosed by incisional biopsy. Surgical treatment was performed with the use of rigid osteosynthesis. Removal of osteosynthesis plate was decided 6 months postoperatively to avoid alteration of mandibular growth and development. Postoperative control was indicated for 5 years. Conclusion: The use of rigid osteosynthesis was adequate to reinforce the mandibular bone defect produced during the surgical treatment of large complex odontoma (AU)

ABSTRACT

Durante el crecimiento y desarrollo de la cabeza, ésta lo hace en diferentes direcciones y proporciones, habiendo un límite entre la armonía /desarmonía conocido como umbral. Se hace referencia a este concepto, la forma de escribirlo y leerlo por medio de un código que lo simboliza. Objetivo: Poner al alcance de la comunidad médica un código de lectura e identificación de fenotipos craneofaciales sindrómicos y no sindrómicos. Conclusiones: Se considera que este concepto de umbral craneofacial y su código de lectura pueden ser usados en la enseñanza e investigación de la armonía-desarmonía durante el crecimiento y desarrollo de la cabeza, resultando ser de gran utilidad en la comprensión rápida y sencilla de la lectura del fenotipo craneofacial (AU)


During the growth and development of the head, it does so in different directions and proportions, there being a limit between the harmony / disharmony known as threshold. Reference is made to this concept, the way of writing it and reading it by means of a code that symbolizes it. Objective: To put within reach of the medical community, a code of reading and identification of syndromic and non-syndromic craniofacial phenotypes. Conclusions: It is considered that this concept of a craniofacial threshold and its reading code can be used in the teaching and research of harmony / disharmony during the growth and development of the head, being very useful in the quick and easy comprehension of the reading of the craniofacial phenotype (AU)

ABSTRACT

O objetivo deste ensaio clínico randomizado foi comparar os efeitos dentoesqueléticos associados ao tratamento da má oclusão de Classe II de Angle 1a divisão com propulsores mandibulares removíveis, fixos e associados à ancoragem esquelética. Após o exame de 1039 indivíduos, foram incluídos 34 pacientes Classe II de Angle, 1a divisão com overjet ≥ 6 mm, de 10 a 14 anos, no pico do surto de crescimento puberal e sem tratamento ortodôntico prévio. Estes pacientes foram distribuídos de forma randomizada em três grupos: (TB) tratado com aparelho removível de Twin Block (n = 13); (HAD) tratado com aparelho fixo cimentado de Herbst (n = 11); e (HAE) tratado com aparelho de Herbst associado a dois mini-implantes mandibulares (n = 10). Foram analisadas tomografias de feixe cônico antes da instalação do propulsor (T1) e após 12 meses de tratamento (T2). No programa ITK-SNAP®, foram construídos modelos 3D da maxila, mandíbula, incisivos centrais e 1os molares. No programa Dolphin Imaging®, foram gerados modelos 3D totais do tecido duro de T1 e T2, com orientação de cabeça padronizada e registro na base craniana anterior. O Modelo de Transferência de Coordenadas (MTC) foi obtido a partir do recorte padronizado do tecido mole de T1, nos planos axial, sagital e coronal. No programa Geomagic Qualify®, os modelos 3D parciais foram alinhados à posição espacial dos modelos 3D totais orientados e registrados. Foi utilizado um método automático para determinar os pontos centroides, que representaram a posição espacial de todas as estruturas anatômicas avaliadas. Os deslocamentos dos centroides entre T1 e T2 foram analisados no sistema de coordenados cartesiano elaborado com base no MTC. Para análise do crescimento maxilar e do mento foi utilizado o registro craniano; para a movimentação dentária e crescimento condilar, foram adotados os registros regionais em maxila e mandíbula. O teste de Wilcoxon avaliou as diferenças intra-grupo e o teste de Kruskal Wallis analisou as diferenças intergrupos das alterações esqueléticas e dentárias. Foram comparados os índices de sucesso na correção da sobressaliência e relação molar. Foi detectada restrição de crescimento anteroposterior da maxila (HAE: -0,26 ± 2,88 mm; TB -0,25 ± 0,66 mm; HAD: 0,18 ± 0,87 mm) e crescimento mandibular anteroposterior (HAE: 4,21 ± 2,96 mm; HAD: 3,49 ± 3,74 mm; TB: 1,24 ± 3,36 mm). No arco superior, foi observada mínima movimentação de incisivos e 1os molares (≤ 1 mm nos 3 planos). Os principais efeitos dentários ocorreram no arco inferior, na perda de ancoragem (HAD: 1,73 ± 0,64 mm; TB: 1,45 ± 1,13 mm; HAE: 1,07 ± 0,23 mm) e mínima projeção dos incisivos centrais (HAE: 0,93 ± 0,08 mm; HAD: 0,54 ± 0,36 mm; TB: 0,27 ± 0,18 mm). Quanto à eficácia, a maior correção do overjet foi obtida pelos grupos HAD (-4 ± 1,65 mm) e HAE (-4 ± 1,47 mm), seguidos pelo TB (-3 ± 3,25 mm). Na relação molar, a maior eficácia foi do grupo HAE (100% de sucesso, sobrecorreção de 85,7%), seguido pelo HAD (100% de sucesso, sobrecorreção de 70%), e TB (55,6% de sucesso). Concluiu-se que o aparelho de Herbst associado aos mini-implantes obteve maior eficácia na correção predominantemente esquelética da má oclusão de Classe II 1a divisão, do que os aparelhos de Herbst e Twin Block, após 12 meses de tratamento(AU)


The aim of this randomized controlled trial was to compare the dentoskeletal effects associated to the treatment of Angle Class II 1st division with removable, fixed and skeletal anchored functional appliances. After the clinical examination of 1039 individuals, 34 patients with Angle Class II, 1st Division, overjet ≥ 6 mm, 10 to 14 years old, at the peak of the pubertal growth spurt and no history of orthodontic treatment were included. These patients were randomized into three groups: (TB) treated with removable Twin Block appliance (n = 13); (HAD) treated with fixed cemented Herbst appliance (n = 11); and (HAE) treated with Herbst appliance associated to two mandibular mini implants (n = 10). Cone-beam CT scans were obtained before (T1) and after 12 months of treatment (T2). In the ITK-SNAP® software, 3D models of the maxilla, mandible, central incisors and 1st molars were built. In the Dolphin Imaging® software, full 3D models of T1 and T2 hard tissue were generated, with standardized head orientation and registration in the anterior cranial base. The Coordinate Transfer Model (CTM) was obtained from the T1 soft tissue, with standardized slices in the axial, sagittal and coronal planes. In the Geomagic Qualify® software, the partial 3D models were aligned to the spatial position of the oriented and registered full 3D models. An automatic method was used to determine centroid points, which represented the spatial position of all evaluated anatomical structures. Centroid displacements between T1 and T2 were analyzed using the Cartesian coordinate system based on the CTM. The cranial register was used to analyze the growth of maxilla and chin; to measure tooth movement and condylar growth, the regional register in the maxilla and mandible were adopted. The Wilcoxon test assessed intra-group differences and the Kruskal Wallis test analyzed intergroup differences in skeletal and dental changes. Success rates for overjet correction and molar relationship were compared. Anteroposterior maxillary growth restriction was detected (HAE: -0.26 ± 2.88 mm; TB -0.25 ± 0.66 mm; HAD: 0.18 ± 0.87 mm) and also anteroposterior mandibular growth (HAE: 4.21 ± 2.96 mm; HAD: 3.49 ± 3.74 mm; TB: 1.24 ± 3.36 mm). In the upper arch, minimal movement of incisors and 1st molars was observed (≤ 1 mm in the 3 planes). The main dental effects occurred in the lower arch, in the anchorage loss (HAD: 1.73 ± 0.64 mm; TB: 1.45 ± 1.13 mm; HAE: 1.07 ± 0.23 mm) and minimal projection of central incisors (HAE: 0.93 ± 0.08 mm; HAD: 0.54 ± 0.36 mm; TB: 0.27 ± 0.18 mm). The highest overjet correction was obtained by the HAD (-4 ± 1.65 mm) and HAE (-4 ± 1.47 mm) groups, followed by TB (-3 ± 3.25 mm). In the molar relationship, the highest efficacy was in the HAE group (100% successful, 85.7% overcorrected), followed by HAD (100% successful, 70% overcorrected), and TB (55.6% successful). It was concluded that the Herbst appliance associated with mini-implants had greater efficacy in predominantly skeletal correction of Class II 1st division malocclusion than the Herbst and Twin Block appliances after 12 months of treatment(AU)

ABSTRACT Objective: To evaluate topographic and temporal aspects of premaxillary bone and premaxillary-maxillary suture, since they are fundamental anatomical elements little explored clinically. Methods: 1,138 human dry skulls were evaluated, of which 116 (10.19%) of the specimens were children, and 1,022 (89.81%) were adults. The skulls were photographed and the percentage of premaxillary-maxillary suture opening was determined. Subsequently the data were tabulated and submitted to statistical analysis, adopting a level of significance of 5%. Results: The progression of premaxillary suture closure from birth to 12 years of age was 3.72% per year. In 100% of the skulls up to 12 years, the premaxillary-maxillary suture open in the palatal region was observed, while 6.16% of adults presented different degrees of opening. Conclusions: The premaxilla exists in an independent way within the maxillary complex and the presence of the premaxilla-maxillary suture justifies the success of anteroposterior expansions to stimulate the growth of the middle third of the face, solving anatomical and functional problems.


RESUMO Objetivo: avaliar aspectos topográficos e temporais do osso pré-maxilar e da sutura pré-maxilar/maxilar, por serem elementos anatômicos fundamentais pouco explorados clinicamente. Métodos: foram avaliados 1.138 crânios secos humanos, sendo 116 (10,19%) dos espécimes crianças e 1.022 (89,81%) adultos. Os crânios foram fotografados e determinou-se a porcentagem de abertura da sutura pré-maxilar/maxilar. Posteriormente, os dados foram tabulados e submetidos a análise estatística, adotando-se nível de significância de 5%. Resultados: a progressão de fechamento da sutura pré-maxilar/maxilar do nascimento aos 12 anos de idade foi de 3,72% ao ano. Em 100% dos crânios até 12 anos, observou-se a sutura pré-maxilar/maxilar aberta na região palatina, enquanto 6,16% dos adultos apresentavam diferentes graus. Conclusões: a pré-maxila existe de forma independente dentro do complexo maxilar e a presença da sutura pré-maxilar / maxilar justifica o sucesso de expansões anteroposteriores para estimular o crescimento do terço médio da face, solucionando problemas anatômicos e funcionais.

Introducción: Los problemas posturales se inician, en la mayoría de los casos, en la infancia, por lo tanto en período de crecimiento una actitud postural alterada compromete el equilibrio cefálico y la posición de la mandíbula pudiendo originar malocluciones. Objetivo: Analizar si existe relación entre el ángulo cráneo-cervical y el patrón esqueletal en una población infantil. Material y métodos: Se evaluaron 70 pacientes con telerradiografía de cráneo de perfil con exámenes cefalométrico de Ricketts con patrón esqueletal de clase I, II y III (Campo II relación cráneo-mandibular, factor 7 convexidad) y el cefalograma de Rocabado, utilizando el ángulo cráneo-vertebral. Resultado: Tanto el sexo como el patrón esqueletal son significativos, su interacción no lo es. A su vez, la edad no resulta significativa. Conclusión: En el presente estudio se encontró que existe relación significativa entre el ángulo cráneo-cervical y el patrón esqueletal (AU)


Introduction: Postural problems begin in most cases, in childhood, therefore during growth an altered postural attitude compromises the cephalic balance and jaw position causing posible malocclucion. Objective: It is to present the correlation between the skull and cervical angle skeletal pattern in a child population. Material and methods: 70 patients were evaluated with teleradiography cephalometric profile tests Ricketts skeletal pattern with class I, II and III (Field II craniomandibular relationship, Factor 7 convexity) and cephalogram Rocabado, using the craniovertebral angle. Result: Both sex and skeletal pattern are significant and their interaction is not. In turn, age is not significant. Conclusion: In the present study it was found that there is a statistically significant relationship between the cranial-cervical angle, and the skeletal pattern (AU)

ABSTRACT

La técnica ortodóncica de baja fricción utiliza brackets pasivos de autoligado con una serie de arcos superelásticos que mantienen las fuerzas aplicadas sobre las piezas dentarias a nivel de la "biozona o zona fuerza óptima". La utilización de gomas intermaxilares ligeras (2 onzas o 56,68 g) en estos tratamientos, desde etapas iniciales, sobre todo en pacientes en crecimiento, permite obtener resultados muy favorables en cuanto a la resolución de la maloclusión de Clase II. Objetivo: evaluar el tratamiento de Ortodoncia de un paciente Clase II esqueletal con sobremordida, sin realizar exodoncias de premolares y utilizando elásticos intermaxilares en fases iniciales de tratamiento. Materiales y método: se presenta el caso clínico de un paciente de sexo masculino de 11 años, de Clase II esqueletal con sobremordida y apiñamiento dentario anterior superior e inferior moderado, con su diagnóstico y evaluación completos pre y post tratamiento y control y seguimiento al año. El tratamiento se realiza con brackets autoligantes (técnica de Damon). Resultados: se obtiene la corrección completa de la Clase II y la sobremordida, alcanzando los objetivos deseados de estética facial y función, en oclusión y desoclusiones, con buen desarrollo de las basales y conservación de la altura de las corticales alveolares a nivel de los caninos. Conclusión: Mediante los recursos terapéuticos disponibles en la actualidad y evaluando al niño en etapas tempranas, podremos combinar diferentes tratamientos para alcanzar los objetivos propuestos en menor tiempo y con óptimos resultados en una maloclusión de Clase II con sobremordida con biotipo mesofacial (AU)


The low friction orthodontic technique uses passive self-ligating brackets with a series of superelastic arches that maintain the forces applied to the dental pieces at the "Biozone or Optimal Force Zone". The use of inter maxillary light elastics (2 ounces or 56.68 yr.) from the initial stages in growing patients, yields very favorable results in terms of correction of Class II malocclusion. Objective: To assess the orthodontic treatment of a skeletal Class II patient with overbite, without extraction of premolars and using elastic in the early stages of treatment. Materials and methods: an 11 year old male skeletal Class II patient with overbite and moderate crowding in upper and lower anterior teeth, with complete diagnostic and evaluation before and after treatment and with annual control. For the treatment, self-ligating brackets (Damon technique) were employed. The results: Total correction of Class II condition and overbite was obtained. The desired goals of facial aesthetics and correct function were attained in occlusion and desocclusion, as well as development of the osseous basis and conservation of the canine alveolar cortical height. Conclusion: Through the employment of therapeutic resources available at present and following up the child through his developmental stages, it is possible to combine different treatments to achieve the proposed objectives in a shorter time with excellent results in mesofacial type patients with Class II malocclusion and deep overbite (AU)

ABSTRACT

The aim of this study was to present a case series of the orthognathic treatment of facial asymmetry due to temporomandibular joint (TMJ) ankylosis and to characterize the current treatment modalities through a literature review. Four patients who presented with facial asymmetry due to TMJ ankylosis between 2010 and 2014 were included in this study. TMJ ankylosis was surgically treated before bimaxillary surgery with advancement genioplasty in some of the cases. In 2 cases, 3-dimensional (3D) models were used for diagnosis and treatment planning, as 3D models are very important tools for planning surgical maneuvers. Aesthetically pleasant facial symmetry and a good facial profile were obtained in all the cases.

Abstract The aim of this article was to evaluate the influence of biological and sociobehavioral factors on the development of malocclusions in children. This is a cross-sectional study nested in a cohort of 350 children who participated in a community intervention study. After six years, an active search identified 290 children with occlusal abnormalities. Socioeconomic status, linear growth, and nutritional status of the child at birth and at the time of measurement, as well as dietary and sucking habits, were investigated. Malocclusions were evaluated across three spatial planes - anteroposterior, vertical, and transverse. The prevalence of these anomalies was 64.5%, primarily in the anteroposterior plane. The logistic regression analysis revealed that pacifier use for 60 months or more, stunting as measured at age six, and reduced gain in head circumference from birth to six years of age, were significantly associated with the development of malocclusion in childhood. Head circumference and linear growth are associated with occlusal anomalies in infants, independent of pacifier sucking.

In compressed dental arches with mild to moderate crowding, space can be gained during the alignment phase to help improve tooth location. The aim of this study was to compare transverse measurements before and after the alignment stage in two groups: Group A treated with passive selfligating brackets, (Damon System) and Group B with conventional brackets (Roth philosophy). The change in transverse diameter between teeth was measured on dental casts taken before and after orthodontic alignment. Twenty four patients of both sexes aged 13 to 36 years, with moderate tooth crowding (4 to 6 mm), were treated. They all received treatment at the Department of Orthodontics, School of Dentistry, University of Buenos Aires. Patients were distributed randomly into two groups of 12. No extraction was performed until the end of the alignment. Dental casts were measured before and after alignment. The distance between upper first premolars increased more in Group B (p = 0.008), and the distance between canines was higher in Group B, with statistically significant difference (p < 0.01). Both systems enable tooth alignment by increasing transverse diameter of the arches in patients with mild to moderate crowding. The greatest transversal development occurs in the interpremolar area in both techniques and is significantly higher with selfligating orthodontics. Intercanine distance increases significantly with conventional orthodontics compared to selfligating orthodontics (AU)


En las arcadas dentarias comprimidas, con apiñamiento dentario de leve a moderado, es posible obtener ganancia de espacio que favorezca la ubicación de las piezas en la fase de alineación por remodelación alvéolodentaria de las arcadas. El objetivo de este trabajo fue comparar las diferencias pre y post tratamiento en la etapa de alineación de los grupos A: brackets autoligables pasivos, (Sistema Damon) y grupo B: brackets convencionales (Filosofía de Roth). Se evaluaron las variaciones en el diámetro transversal, antes y después de alinear ortodón cicamente los dientes, utilizando modelos de estudio. Participaron 24 pacientes de ambos sexos, entre 13 y 36 años con apiñamiento dentario moderado (entre 4 y 6 mm) tratados en la Cátedra de Ortodoncia de la Facultad de Odontología, Universidad de Buenos Aires. Los pacientes fueron distribuidos al azar en dos grupos de 12 pacientes cada uno. No se realizaron exodoncias hasta finalizar la alineación. Se midieron los modelos pre y post alineación. La distancia entre primeros premolares superiores aumentó en el grupo A más que en el B (p=0.008) y entre caninos fue mayor en el grupo B, con diferencia estadísticamente significativa (p< 0,01). En pacientes con apiñamiento leve a moderado, la expansión con ambos sistemas permite la alineación dentaria por aumento del diámetro transversal de las arcadas fundamentalmente en el área premolar. El mayor desarrollo transversal se produce en el área premolar para ambas técnicas y es significativamente mayor con ortodoncia autoligable. En ortodoncia convencional, aumenta significativamente la distancia intercanina en comparación con la autoligable (AU)

ABSTRACT Objective: The aim of this systematic review was to analyze the short and long-term spontaneous dentoalveolar changes of the mandibular dental arch after slow (SME) or rapid (RME) maxillary expansion in the mixed and early permanent dentitions. Methods: An electronic search was performed in the following databases: PubMed/Medline, Cochrane Library, Scopus, Embase and Web of Science. Eligibility criteria for article selection included randomized controlled trials and prospective studies written in English, with no restriction of year of publication, involving patients who underwent SME or RME during the mixed or early permanent dentitions. A double-blind search of articles was performed by two reviewers. Initially, the title and the abstract of the studies were read, and their references were also hand-searched for possible missing studies. A methodological quality scoring scale was used to analyze the selected articles. Results: The search retrieved 373 articles, but only 6 were selected for review after application of the eligibility and exclusion criteria. Non-clinically significant spontaneous dentoalveolar changes of approximately 1mm were found in the mandibular dental arch in the short and long-term, after slow or rapid maxillary expansions. Furthermore, no significant differences were found between treated and control groups. Conclusions: There is enough evidence to conclude that negligible short and long-term spontaneous dentoalveolar changes tend to occur in the mandibular dental arch after SME or RME in the mixed and early permanent dentitions. More randomized studies with appropriate control group are required to better evaluate this issue.


RESUMO Objetivo: o objetivo da presente revisão sistemática foi analisar as alterações dentoalveolares espontâneas, em curto e longo prazos, após a expansão lenta (ELM) ou rápida (ERM) da maxila, durante a dentição mista e permanente jovem. Métodos: uma busca eletrônica foi executada nas seguintes bases de dados: PubMed/Medline, Cochrane Library, Scopus, Embase e Web of Science. Os critérios de elegibilidade para a seleção dos artigos incluíram ensaios clínicos randomizados controlados e estudos prospectivos escritos em língua inglesa, sem restrição do ano de publicação, envolvendo pacientes que foram submetidos a ELM ou ERM durante a dentição mista ou permanente jovem. Dois revisores realizaram, de forma independente, uma busca por artigos. Inicialmente, o título e o resumo dos artigos foram lidos, e uma busca manual foi realizada nas referências dos artigos selecionados, a fim de se avaliar possíveis estudos não encontrados. Resultados: trezentos e setenta e três artigos foram encontrados com a busca, porém apenas seis foram selecionados para a revisão após a aplicação dos critérios de inclusão e de exclusão. Alterações dentoalveolares espontâneas sem significância clínica de aproximadamente 1 mm foram encontradas na arcada dentária inferior em curto e longo prazos, após a expansão lenta ou rápida da maxila. Além disso, não foram observadas diferenças significativas entre os grupos tratados e os controles. Conclusão: existe evidência suficiente para se concluir que alterações dentoalveolares espontâneas sem significância clínica tendem a ocorrer na arcada dentária inferior em curto e longo prazos após a ELM e ERM na dentição mista e permanente jovem. Mais ensaios clínicos randomizados com grupos controles adequados são necessários para melhor avaliar essa questão.

Durante la infancia es muy frecuente encontrar alteraciones del desarrollo,las cuales derivan de una defi ciente formación de las estructurasanatómicas durante la embriogénesis. Puede encontrarse un sinnúmerode alteraciones del desarrollo que afectan la región bucal y maxilofacial.La gran mayoría de estas alteraciones han sido catalogadas como síndromes de orden genético; sin embargo, no todas pueden describirse como tales, pues existen anomalías del desarrollo que aparecen como consecuencia de una deficiente embriogénesis de la región facial, provocando alteraciones anatómicas y funcionales, pero que se apartan de componentes genéticos y cromosómicos específi cos. La secuencia malformativa de Pierre Robin es una de ellas, ya que esta condición es producida por una afección inicial, de la cual derivarán otras afeccionesadicionales a nivel del paladar y de la mandíbula que ocasionarán en elpaciente dificultad para la alimentación y respiración. Debido a que las alteraciones de esta condición afectan directamente la cavidad bucal,es crucial que el odontólogo se encuentre familiarizado con esta anomalía. El objetivo del presente artículo es describir las característicasque configuran esta entidad nosológica mediante la exposición de un caso clínico y revisión de la literatura.


During childhood, it is frequent to find development disorders whichare linked to the weak formation of anatomic structures duringembryogenesis. It is possible to find a plethora of developmentdisorders that aff ect the oral and maxillofacial region. The majorityof these disorders has been classifi ed as genetic malformations butnot all can be described as such. That is because some developmentdisorders appear as a result of a defi cient embryogenesis of the face,producing thus anatomic and functional malformations but that standapart from genetic and chromosomic specifi c components. The Pierre Robin sequence is one of them, given that this condition is producedby an initial disorder, followed by other disorders in the palate andjaw; provoking alimentary and breathing disabilities in the patient.Due to these disorders and their impact on the mouth, it is crucial thatdentists be familiarized with such anomalies. The aim of this article isto describe the key characteristics that defi ne this disease through thepresentation of a clinical case and a literature review.

ABSTRACT

La importancia de la lactancia materna está más que probada y publicitada. Sin embargo, poca difusión tienen los beneficios de amamantar (tiempo y posición) para lograr un correcto desarrollo de los maxilares, prevenir la adquisición de malos hábitos y obtener una correcta oclusión. La inadecuada relación dentaria no pone en riesgo la vida del infante, pero ya mundialmente es considerado como un problema de salud pública por su prevalencia e incidencia. Se realizó un estudio para evaluar el grado de información que manejan las embarazadas platenses respecto al tema y se buscó verificar si los resultados estaban relacionados o no con la edad y grado de escolaridad de ellas.
